The expectation that we take personal responsibility for our health has become a truism. So why, when we often know what to do, is it still so hard to put this knowledge into practice? This conference will take a provocative look at the economic, behavioral and cultural factors which influence our health behaviors and decisions and offer examples that we can put to use to improve our own health and the health of our communities.

Who should attend:
Anyone interested in gaining a better understanding of the forces that shape health behavior and decision-making should attend. Health care practioners and public policymakers, systems leaders, payers and funders, health care educators and advocates and researchers should all find the speakers and panel
discussion informative and thought provoking.

Implementing the Transitional Care Model

Our next webinar, "Implementing the Transitional Care Model," will be held on March 2, 2010. Mary Naylor, Ph.D., R.N., FAAN from the University of Pennsylvania will provide an overview of her transitional care model (TCM) and Randall Krakauer, M.D. from Aetna will discuss the model's implementation.


The Transitional Care Model (TCM) provides comprehensive in-hospital planning and home follow-up for chronically ill high-risk older adults hospitalized for common medical and surgical conditions.
