Implementing the Transitional Care Model

Our next webinar, "Implementing the Transitional Care Model," will be held on March 2, 2010. Mary Naylor, Ph.D., R.N., FAAN from the University of Pennsylvania will provide an overview of her transitional care model (TCM) and Randall Krakauer, M.D. from Aetna will discuss the model's implementation.

The Transitional Care Model (TCM) provides comprehensive in-hospital planning and home follow-up for chronically ill high-risk older adults hospitalized for common medical and surgical conditions.

Our Collaboration

The Robert Wood Johnson Foundation's Interdisciplinary Nursing Quality Research Initiative and the Donaghue Foundation have formed a collaboration to explore the challenges in translating research into practice.

On July 8 and 9, 2009, we hosted a health funders forum: "Creating Opportunities and Breaking Down Barriers to Adoption." This meeting brought together funders from a variety of organizations to discuss opportunities and challenges associated with the adoption of research findings. A work group formed after the meeting continues to discuss ideas for funders.
